Hypersensitivity Reactions:

All pseudoallergies are a form of hypersensitivity reactions, but not every hypersensitivity reaction is a pseudoallergy.

Hypersensitivity is the broader category, referring to any exaggerated or inappropriate immune response.

Pseudoallergy is a specific type of hypersensitivity in which symptoms resemble those of a true allergy, but the underlying mechanism is not IgE-mediated.

Pseudoallergies are also referred to as non-allergic hypersensitivity reactions or pseudoallergenic drug reactions.

Instead, these reactions can be triggered by various mechanisms, including direct mast cell activation, bradykinin-mediated responses, leukotriene-mediated responses, or the drug itself acting directly on inflammation.

Picture this: a hypersensitive response occurs when your immune system mistakenly perceives something harmless as a significant threat to your well-being, leading to an overreaction that can cause all kinds of health problems.

While this non-allergic hypersensitivity reaction shares similar symptoms with true allergies, the underlying mechanism is different.

Think of the body's immune system as a security guard whose job is to identify and combat intruders, such as viruses or bacteria.

Just like a Carpa reaction, this new guard perceives something entirely harmless, such as a speck of dust (pollen), a friendly delivery person (certain foods and drinks), or a harmless vitamin pill (a chemical).

Instead of just checking it out and realizing there's no danger, the security guard immediately goes into full attack mode.
It sounds all the alarms, unleashes the guard dogs, and sprays everything with pink pepper spray!This "overreaction" is a hypersensitive response.

The outcome can range from a mild, annoying reaction, like getting a rash (the pepper spray causing minor irritation), to something very dangerous and life-threatening, where the security guard's extreme actions cause the entire building to collapse!

Treatment for hypersensitivity reactions depends on the type and severity of the reaction.

For mild reactions, antihistamines like Aerius may be sufficient, and Locobase LPL cream can also be helpful.

For chronic itching (pruritus), edema, and contrast reactions, Tavegyl injection can be considered. (I am allergic to the Tavegyl tablets)

Severe reactions, such as anaphylaxis, require immediate medical attention, including the administration of epinephrine.

Immediate reactions:

These typically occur within an hour of administration and can include symptoms like hives, facial swelling, respiratory distress, rapid heart rate, and a drop in blood pressure, which can potentially lead to death.

Among the agents most frequently involved in immediate allergic reactions are: Hydrocortisone, prednisone, methylprednisolone, and triamcinolone acetate.

Non-immediate reactions:
These are more common and can appear up to days after administration. They often present as a disseminated rash, but it can escalate to a life-threatening reaction and include chronic eczema, mainly when used in conjunction with topical corticosteroids.

Important Considerations:
There can be significant cross-reactivity between different corticosteroid drugs, meaning a reaction to one may indicate a potential response to others, especially if they share similar chemical structures.

Diagnosis:
Hypersensitivity reactions can be challenging symptoms that mimic conditions for which corticosteroids are given, like shock. status asthmaticus. Skin testing and provocative challenges can help in diagnosing these reactions.

Treatment:
If an allergic reaction is suspected, the corticosteroid should be discontinued, and an alternative preparation that can be tolerated by the patient should be used.

Emergency Situations:
Severe allergic reactions, such as respiratory distress, sudden swelling of the lips, mouth, or throat, severe abdominal pain, or severe dizziness, must be attended to immediately.
